ADD shares some symptoms with other disorders, so you might be better off seeing how the doc progresses with his diagnosis. In preparation, you may want to print out some material on ADD that you feel represents your particular symptoms, but instead of guiding your doc towards ADD as a diagnosis...I would see where he/she goes with it first. In case he/she asks you why you're there, mentioning a ADD specifically may make them think that you're self-diagnosing. You might want to just rattle off the symptoms that you're experiencing, and let the doc make the leap to a Dx. Worst case, you have some printed materials as backup to share with the doc.
