My psychiatrist suggested I apply for Disability for my ADD condition. She feels I am unable to work and should not even look for work until we get it under control. She has never recommended this to a patient before so has no experience in qualifying. I am supposed to fill out 3 sections of the form and then give it to her where she fills out 2 sections. I have not begun to fill out the form. I'm very intimidated by it. One question that baffles me is "On what date did your disability begin"

EXCELENT IDEA BARBETTE....This peer coaching forum is for you only...(the focus is on you only)..."On what date did your disability begin"? I would think it would be either 1) when you were diagnosed OR 2) when you first had symptoms.... WHAT do others think????

I was diagnosised over 6 months ago. and have symptons since childhood. I'm thinking either choosing. 1) the day I was fired, meaning I was not functioning well enough to work. or 2) The date my Dr. suggested maybe I was unable to work due to my condition.
My next appt. with Dr. is July 16. I will discuss it with her.

Okay, but I would take the date which came earliest....whether it was the day you were fired (meaning not well enough to work) OR the date your M.D. suggested you were unable to work due to condition....reason I say this is it makes you look as if you've been sicker -- LONGER...(and so more likely to get the disability benefits).....know what I mean? anyway, just my thought...yup, ask the doc...:)

I decided against filing for State Disability. I think it is better to try and find a job I can do. I don't want to have the attitude I am disabled. Also it may be harder to get a job if I am on disability.
I talked it over with my Dr. and she agreed. She also increased my medication.

It sounds like you feel good about your decision.....am I right? So do you have ideas about what kind of job you are able to do?:)
