Here's the most recent draft. It seems like too much detail to me but I don't know.


We request the school district evaluate our son, STUDENT, for special education services under the Individuals with Disability Education Act and any disabling condition under Section 504 of the Rehabilitation Act.

We are concerned about STUDENT'S progress in school this year. He is diagnosed with Attention Deficit/Hyperactivity Disorder (ADHD) and anxiety both of which are impacting his school performance and behavior. These diagnoses were communicated in writing to the school in August 2009 and again in March 2013.

STUDENT has had difficulties focusing and remaining on task which has led to him not fully understanding all concepts and being behind in his assignments as well as receiving poor grades. He has particularly struggled in mathematics this term where he previously had little to no concerns. Additionally, STUDENT struggles with reading skills. He reads very slowly and has difficulty completing and comprehending tasks requiring lengthy reading assignments.

These issues have led to an increased level of frustration for STUDENT. The school performance issues he has had this year have increased STUDENT'S anxiety level to the point where he has been unable to attend his classes on a consistent basis since the beginning of February 2013. He was absent 12 consecutive days from February 4 through February 18, 2013. Since that date, STUDENT has had variable success where some days he attended without issue, some days he required time in the office to acclimate before going to class and still other days he needed to return to the office at some point during the day to calm himself before returning to class. STUDENT'S anxiety responses have also resulted in him being suspended 3 times in March 2013 for a total of 8 days.

Kindly provide us with a Permission to Evaluate form so they we can move forward with the process to provide STUDENT with the assistance he needs to succeed.

I don't think this is over-detailed at all. You have to assume that someone who has never heard of your son, and who can't quite remember which school he goes to or what's happening here, is the one with the power to help you.

Oh - in the "Kindly provide..." paragraph near the end, it says "so they we" - it should be "so that we".

Thanks for your feedback! I didn't show it to the dr today but we did discuss it. He said either way was fine and a 2 sentence request was even adequate.
