Hi everyone,

I've been on concerta 54/day for 3 months now, and found it pretty good. Lately it's a little bit less effective, and certain side effects (like appetite suppression) have improved.

My rapid-cycling bipolar has been stable-ish for a few years. I haven't needed meds for it since then, but I've had some brief mild-moderate episodes in both directions. I think I cycle at a very low level (almost imperceptibly) most of the time, and usually only realise with hindsight that I've been a little up or down for a few weeks.

I had a moderate hypomanic episode about a month ago, seemingly triggered by physical illness. Since then I've had mild cycling, and some very brief (a few days) moderate-severe downs. It's been improving, but on days when I feel so-so, I find that once my ADHD meds kick in I can feel quite down and I get the heavy-paralysed feeling I associate with being severely down (depression for me is almost more physical than mood-based, energy levels and appetite etc are my main clues.)

I'm not sure what to do. I had a med holiday yesterday and felt chipper all day. Today I woke up fine, then took my meds, and an hour or so later I'm heavy, morose and irritable. :( Do I have low-level depression that's being exaggerated by the concerta? I'm really perplexed.

I could never survive bipolar without treating it with medication.
