How would I go about getting evaluated for learning disabilities?

You would see a mental-health professional such as a psychologist.

Ask your doctor or psychiatrist.

My reply was short on useful information...

Whoever you see, your part of the process is straightforward. You go to them and you say "I really think I have some kind of learning disability" - they say "Why?" - you tell them -

or else they say "Pffft, no you don't", and you say "Please, hear me out on this one, this is what happens..."

A qualified educational psychologist is the best IMO for learning disabilities I was assessed for my other issues in childhood and adulthood by one make sure they are legit can run into the 350.00-400.00 range (though my childhood one was free as done by Nhs, they aren't available on the health service for adults) as most aren't diagnosable by any tick list or talking about the problems though any psych worth his/her stuff should do a comprehensive assessment using all of the psychometric testing data plus info you and/or an informant can provide about your childhood and adulthood. WAIS IV is usually the tool of choice by edu psychologists. Make sure you find one that's an adult specialist as opposed to solely children

The testing process will take around an hour and a half then add into that compiling information about your experiences in every day life and your personal history. It can feel a little too much like being poked and prodded but it's necessary so that they can get an accurate picture of you and your strengths/weaknesses.

Thanks. I guess I will start by going to my schools Counseling office and see what they say. I'm in the US.
I've always struggled really bad with math. I took my last math class ( per algebra ) 3 times before passing and now I have to take intermediate algebrafor the second time :/

You might also see if they're doing any studies in your area. In New York, there's usually a study on just about anything going on, they even pay you to take cocaine! I never did that, but I went to John Jay School of Criminal Justice (I think that's what it is) and watched some tape then had to face a grilling about all the details of it, it was to see how reliable eyewitness testimony is -- I sucked, so you wouldn't want me testifying in court about a robbery I saw!

Sometimes you might be able to get an IQ test and they'll actually pay you! Make sure that they will give you the results though at the end.
