Hey there, i was prescribed dex on saturday, it is now Wednesday and i already feel like i am not taking enough? i was diagnosed with adhd 2 months ago, i tried strattera with minimal success (fatigue, depression, slightly increased focus) so i saw a different psych who could prescribe stims. I was given dexamp which i have studied alot since then.

day 1 i felt pretty good, nothing too major, i took a 5 to begin with and then 2x5 at lunch. i was advised to start on 5 and if i didnt notice any side effects move to 10 morning and lunch.

day 2 i had 2x5 morning and 2x5 lunch, i felt a lot better, no side effects, i was able to focus and my recall was better. i had a great day.

Day 3 i did the same but felt a little slower, kind of tired just put it down to a first night on the meds

day 4 i did the same, felt a bit more tired and my symptoms creeped back a bit, took a bunch of vitamins (i have a large arsenal) and ate better and had a good sleep

day 5, i felt much tireder today, symptoms are only about 50-60% treated

Questions
Do i need to up my dose? i have a very high tolerance to most things.

i was given a script for 200 pills (6x5 a day) and i run a 3 days on 1 day off study schedule to give my body time to negate tolerance to the drug. however i am 5 days straight as i had 2 major assignments to do because i was falling behind pre meds.

Is it normal to have almost all effects were off in 5 days? or is it that i just need to up my dose to maybe 15 in mornin and 15 4 hours later.

i do not get a crash or anything i am pretty happy into the night as i am very healthy (tried everything before meds) i wake up straight away ready to take my meds and tackle the day as before this my life was a challenge.

Advice anyone? it took over a month to see my psych, so calling him isn't an option for me. i have read that people find their own sweet spot, maybe mine is a little higher?

I say give it a couple more weeks on the lower dosages.

50-60% coverage of symptoms is actually pretty good, many people don't even reach 50% symptom coverage on meds.

It's not going to be perfect.

You say you're tired - are you eating proper meals throughout the day? Staying well hydrated? How's your sleep?

Often, people think they need a higher dose when really a lower dose works better.

Trust me - you don't want to take too high a dose. I was once on 70mg of Vyvanse for nearly a year and I was a zombie while it was in my system. It was too high of a dose and it took me a very long time to realize that.

Start low and go slow. Try to find the lowest dose that is therapeutic.

the thing is the first 3 days i had excellent results like other people report, my daily life improved, i felt great for the first time in years. i didnt groan like a 70yr old with arthritis in his knees when i bent over to pick up a sock. i never wanted to do anything and nothing felt fulfilling.
Day 2-3 i felt really good, i started to clean up after myself, i felt accomplished when studying and my libido was increased. i could focus and ignore the crazy thoughts i got while studying. studying still wasn't a cake walk but i could choose to block out the noise and focus.
now thats starting to fade at the same dose? i am getting tireder slowly and my coverage has dropped from 90%ish to 50-40 area. i can focus here and there while on it but not for long. My diet went a bit low day 2 but by day 3 it was improving, i have 1L vege shakes from my ninjabullet filled with the best possible stuff. i dont drink coffee, i dont watch porn, i barely eat sugary ****, i don't drink, i exercise and take a plethora of vitamins.
I have 0 side effects aswell which everywhere states i should up my dose until i have coverage of my symptoms and intolerable side effects and then take the dose below that so i can tolerable side effects and coverage?
honestly, i am healthy as can be, i am a trained personal trainer (i dont work in the field i am studying finance)

Tomorrow i am going to try 15am and 10 4 hours later to see if that works, if not i might try going lower but i would like a few more opinions?

i should reiterate that i have a super high tolerance to things? for instance, i used to smoke pot a long time ago, i didn't have a cap on how much i could smoke and never had issues with it, litterly smoking a quarter to myself once because i had to test it while others where on the floor 5-10 cones in. Drinking when younger i could always hold my liquor really well, etc, etc

Scott, What did you and your Doc agree to in handling the drugs? Specific starting dose (10 mg tid?), how often to reevaluate changing dose (once every week?), how much to increase daily dose by (5 mg?), what range of daily dose (up to 30mg/d?). You have to stay within the agreed boundaries unless you talk to the Doc or tell him what you have planned.

You indicate you are taking 6-5mg tabs/day which is 30mg/d. How was this 30mg split up. (1x/d??, 6 x/d??) You propose doing 15mg twice a day which is 30 mg/d.

As far as dosing you want to find the single dose that best addresses your symptoms. You also need to find out how long that dose lasts for you. (maybe 2 hours, maybe 6) In general, you do this by starting at too low a dose for a week or so. Note, like you have, how well your symptoms are addressed and ALSO how long the dose lasts. For me it is ~3 hours. Then if symptoms are still an issue, bump the dosage a bit for a week. Note symptoms and how long the doses last. Then if symptoms are still an issue, bump the dosage a bit for a week. Also adjust how often you take the doses based on how long each dose has lasted.(not on how well your symptoms are doing.) Rinse and repeat until your dose is too high or all of your symptoms are addressed. (Back off a bit if you went too high.). HTH, -LN

Dont worry about tolerance. Its rare and your tolerance to other things has nothing to do with whether or not you would have one with dex.

Again, people are always freaking out and worrying about tolerance. You always have adhd and it impairs your life outside of school and work right? So why wouldnt you treat it the same consistent way everyday?

What you have to look at is whether any/all/some of your symptoms are controlled and not how you feel. You may need a higher dose. I take 50mg a day. But if you dont give each dose change a consistent schedule and a consistent amount of time you wont be able to figure out what is best for you.
And I dont understand....why cant you call your doctor?

i'm new to dex too, my dr gave me little advice and sounds like you went through all same process as me.

I ended up with 2x5mg tabs in the morning until lunch then another 2x5mg which is good in my opinion for what i needed. but at 7-9 i would be irritable and i was trying to take care of three kids coming down, so i added a 5th 5mg at 3-4 gets me to 11.

i had two very nice weeks of getting tired at 11pm and going to bed now i rarely sleep. you just need to be consistent and work with a dr who can help you decide on the dose that works for you and your needs.
