View Full Version : Dexamphetamine dosage experience in NSW Australia

Hello folks,

FYI, I'm a 57 year old male, diagnosed in Feb 2016 and had to switch to Dex as the Ritalin Rollercoaster nearly brought about my demise.

Now...the story...

For a couple of months after the Ritalin disaster (even though it worked SO well) I was on 6 x 5mg Dex per day (6am-10am-2pm) As they are not working as best they can I am now taking 9x5mg per day at the same 4 hour interval. There is a marked improvement however I will be moving upwards to 4x5mg per day as I experimented with this does for a few days.

I did inform my Psych Dr about this and she was most understanding of my “self research” and agreed that a higher dose will be needed , however here in New South Wales (Australia) the most a Psych Dr can prescribe is 30mg per day unless they obtain a second opinion from another Psych Dr who also specialises in adult ADD.

I have seen this 2nd Dr and he was fine with my case and has written to the first Dr and to the Ministry of Health supporting the dosage increase.

Some interesting facts were mentioned by the 2nd Dr which I will share here.

1) No matter what the amount of Dex I (we) take, on average %50 of it goes down the toilet within the first 2 hours. So, take 20mg, we actually have 10mg working etc etc.

2) The limit of 30mg here in NSW and perhaps the rest of the states in Australia was proposed about 30 - 40 years ago by a group of beaurocrats sitting around a table with little or no clinical expertise.

Due to the increased diagnosis of ADD in adults the medical community are now lobbying to have this limit either lifted or removed and left in the hands of trained Drs who specialise in this condition.
For the record for those not living in Australia that read this post, it was cheaper for me to get a one off 2nd opinion at $497 than to have the long acting medication “Vyvanse” prescribed as it’s $150 per month. The jars of Dex 5mg IR only cost me about $40 per month.
Sadly, we are very limited in the medications we have available here.

intersting information. Do you know anything else they may look at?

I'm also a bit sick of the extra $500 you get charged after every new dr.

Hi Pilgrim,

I saw my normal Psych yesterday (Wednesday 17th August) and she wants me to stay on the 3x5mg x 3 times per day and see my GP as my blood pressure ramped up significantly on the 4x5mg.

The only other medication she mentioned is the Vyvanse or some no-stimulant ones but she wants to leave those as a last resort as the stimulant meds work so well for me.

Oh and while on the subject of what works, I must say that when I was trialing the 4x5mg dose I felt as "crystal clear" and as functional as to be classified as "normal"

I only felt this normal when I was on the Ritalin at 20mg x 3 times per day.

Anyway, as more information comes to light, I will post it.

Sorry bout the late reply,

Quite a few things have happened since I posted last. Saw the 2 nd psyc dr and he would be my choice. Very dynamic. The other guy knew heaps about ADD but really wasn't interested, can't have that.

The new guy actually said he wouldn't prescribe more than 8 a day, which is cool. Everything else about him is just excellent. This settles me in many ways. I feel more confident.
He has also pushed the Vyvanse, I've been resisting. I will see him soon.

I'll fill you in also.

I am in US and have been on ritalin 20 mg 3x a day for years and years.

It is getting where it doesn't work as well, so I am going to switch to either dexadrine or adderall.

Vyvanse IS extended release dexadrine, so if you like dex, you may like it.

I switched to Vyvanse a few years ago for several months, and I really liked it, but it takes a couple hours to really "kick in" or whatever, also even long after it wore off I couldn't sleep, I was exhausted but about every other night I was awake all night. That was brutal.

But from about 10 am to 5 pm, on days where I had slept, I was SO CLEAR, calm, happy, controlled, etc. But also, friendly, relaxed, fun etc. None of the type A behavior ritalin gives me. There is nothing fun about me on ritalin, I am about shutting up, and getting some stuff done. I'm not warm and snuggly on ritalin like I was on Vyvanse, and like I suppose I'll be on dex.

Do you think for someone stimulant tolerant, who takes ritalin 20 mg 3x a day, dexadrine 10 mg 3x a day would be about right? I started on Vyvanse 60 mg when I tried that, dropped down to 50 mg, and was planning to drop down to 30 or 40 mg with an IR booster in the afternoon as needed, and then my dad died, and I didn't want to deal with it anymore and went back on ritalin.

I don't want to do XR anything cause I work nights and flip flop my schedule all over.

I just started a pill for my blood pressure/anxiety (propranolol), so I want to settle in and get used to it before I mess with my psych meds. I am also on effexor for depression and ambien for sleep as needed.

I was thinking either adderall 20 mg 3x a day, or dexadrine either 10 mg or 20 mg 3x/day.

I am concerned about future "drug shortages" that seem to occur with adderall and dexadrine. (Notice how Vyvanse is never in a shortage? Hmmm.)

I wonder when the patent wears off it, what the "new" old drug will be.

It wouldn't shock me to see dextromethamphetamine reformulated to be abuse proof. Of course, that one will scare everyone....maybe they can take out the h and change the ph to an f....."dextrometamfetamine " ah perfect, that'll fool everyone! Lol.

I really thought cyclert would end up back in an improved form by now.

Well methylphenidate is different then all the amphetamines you mentioned so it would be hard to say what dose would work. And if you are really tolerant to ritalin, you shouldnt be tolerant to amphetamines because they are different.

I hear what you are saying but just in case you need more coverage then the ir would give you I wanted to let you know that adderall xr isnt meant to last more than 6-8 hours. I only get 4-5 because I have absorption issues so I take it multiple times a day.

I think that would be a reasonable goal to get to, but I think you should start out lower.

I did have to go through the adderall shortage a few years ago so I had to flip between the ir/xr.
I would say its not all that typical.

Of course anything in the drug market is possible, but I am hoping the recent attention will keep these companies in check.
