I take a diuretic (thiazide) as well as a stomach acid blocker (omeprazole). In addition, my urine is acidic (I don't take anything for this). I drink two cups of coffee per day.
I take Adderall XR 30 mg twice a day! I thought I might be a fast metabolizer, but how are the above factors affecting my Adderall XR absorption?
Should I ask my cardiologist to consider eliminating the diuretic now that I am on adderall (acts as a diuretic)?
Should I ask my gastroenterologist (for mild GERD) about taking omeprazole only as need, rather than every day?
Should I ask my general practicioner about an urinary alkalizing agent (allopurinol)?
Should I just quit coffee?

My doc has me take Prilosec as needed. I have issues with ulcers if I take an anti-inflammatory (Motrin, etc) but due to a foot injury, I need to take the anti-inflammatories occasionally. I think your doc might give you the thumbs up to take on an as needed basis. Also, have you tried Pepcid Complete? Its OTC, and more expensive than Pepcid, but its fast acting, AND lasts over time too. Its work a shot. It won't mess with your metabolic state like a PPI.
I'd also talk to the doc about the elimination of the diuretic, but on a slow basis. For myself, the adderall acts as a anti-diuretic at night, but the wearing off actually produces a diuretic effect (crazy, huh?!).
Quitting the coffee may help with the acidic effect, but I don't think 2 cups would make a huge difference... Are you drinking any other sodas, or fruit juices?
I'd call your GP and talk it all over with him, as well as your psych. Good luck.

Yes to all the questions about asking your doc, but here's what the Adderall XR prescribing info says about interactions with some related meds:
Andrenergic blockers -- Andrenergic blockers are inhibited by amphetamines.
Antihypertentives -- Amphetamines may antagonize the hypotensive effects of antihypertensives.
Alkalinizing agents -- Gastrointestinal alkalinizing agents increase absorption of amphetamines. Co-administration of ADDERALL XR and gatrointestinal alkalinizing agents, such as antacids, should be avoided. Urinary alkalinizing agens increase the concentration of the non-ionized species of the amphetamine molecure, thereby decreasing urinary excretion. Both groups of agents increase blood levels and therefore potentiate actions of amphetamines.
I wasn't able to nail down if either of your meds fall directly into one of these categories, except the one alkalinizing agent you asked about.

I just checked - allupurinol is not rally an alkalinizing agent, it reduces uric acid. Alkalinizing agents would be potassium citrate, bicarbonate, etc. If my doc puts me on say, potassium citrate, I could reduce the amount of Adderall I take.
I'm taking Benicar with thiazide for blood pressure; perhaps I should switch to regualr Benicar.
