I am trying to locate a symptom checklist for my 14 yr old stepson. His dad has been diagnosed with ADHD and is on Adderall for treatment. We are seriously considering having our son evaluated but may have some problems getting mom to agree. We would like to have our son take the symptom checklist just to get a feel for how many of the symptoms he is actually aware of. It is pretty obvious to us that he more than likely has it, but that is because we live with it daily already. Any suggestions would be greatly appreciated.

I would post a link but, I do not think we are allowed to do so here. Anyways, do a search for "Amen Clinic" on Dr. Amen's site there are several different evaluation tools and I believe there is a checklist for ADD on there too.
