I have been on Concerta with great results for about 6 months. I started at 18mg and then 27mg. The 27mg dose works well but is not very long lasting. I tried the 36mg but it is too much for me at the peak for about an hour or two (I become introverted and over focused).

So my Doctor prescribed me an 18mg dose and then another 2 hours later to give enough dose but to try and level the peak. He told me to play with the timing, try 2 hours, 2.5 hours, 3 hours and see what works best. It works great, but I have noticed fluctuations from one day to the next on effectiveness (I take them 2 hours apart).

Has anyone experienced fluctuations in effectiveness with tiered dosing?
