Check with your doctor.
From the Adderall XR medication guide:
ADDERALL XR® tell your or your child’s doctor about all health conditions (or
a family history of) including:
• heart problems, heart defects, high blood pressure
• mental problems including psychosis, mania, bipolar illness, or depression
• tics or Tourette’s syndrome
• liver or kidney problems
• thyroid problems
• seizures or have had an abnormal brain wave test (EEG)
Tell your doctor if you or your child is pregnant, planning to become pregnant,

also it's a good question for your pharmacist :)
Probably not as far as having negative effects on the kidneys.
All medications have to be used with caution for people that have liver and/or kidney problems since those (2) organs are involved with breaking down and removal from the body.
If someone has a poorly functioning liver or kidney(s) then usually the dosage has to be dropped way lower than normal since the medication will be floating around in the body much longer than normal.

As mentioned, ask doctor and pharmacist. You should have regular blood work to check your liver and kidneys just to be on the safe side.

Agreed.
Also, if it increases your blood pressure, this can definitely impair kidney function. I would advise to avoid any blood pressure increasing drug in someone with kidney disease. (But if dosed/taken correctly, this should hopefully not be an issue)
