During development, Vyvanse was known as NRP104. Its main ingredient is lisdexamfetamine dimesylate. Vyvanse is unique in another way too: it is a prodrug or 'conditionally bioreversible derivative' of amphetamine, the main ingredient in Adderall and Adderall XR. This is a key point: unlike other stimulants it is conjugated to an amino acid, and has to go through the stomach and be digested before it can become active. Therefore there is a much reduced risk that it will be abused, since it cannot be snorted or injected like other ADHD medicines.

There was a great deal of new data presented at the In research presented this week at the 2007 Annual Meeting of the American Psychiatric Association in San Diego, California. The bottom line seems to be that the medicine is both effect and well tolerated. It is going to be a genuinely useful addition to the treatments that we can offer people with ADHD.

Vyvanse (lisdexamfetamine dimesylate) is designed as a capsule for once-a-day oral administration. The chemical designation for lisdexamfetamine dimesylate is (2S)-2,6-diamino-N-[(1S)-1-methyl-2-phenylethyl]hexanamide dimethanesulfonate. The molecular formula is C15H25N3O•(CH4O3S)2, which corresponds to a molecular weight of 455.60. The chemical structure is:

Lisdexamfetamine dimesylate is a white to off-white powder that is soluble in water (792 mg/mL). Vyvanse capsules contain 30 mg, 50 mg and 70 mg of lisdexamfetamine dimesylate and the following inactive ingredients: microcrystalline cellulose, croscarmellose sodium, and magnesium stearate. The capsule shells contain gelatin, titanium dioxide, and one or more of the following: D&C Red #28, D&C Yellow #10, FD&C Blue #1 and FD&C Red #40.

Dextrostat has %25.5 more dextroamphetamine base then Adderall
Dextrostat has %8.8 more (total d-, or l-)amphetamine base then Adderall
When comparing equal doses of Dextrostat and Vyvanse
Dextrostat has 70% vs 30% from Vyvanse sooo Dextrostat has 40% more then Vyvanse

(assuming 20mg of adderall):

7.5mg of d-amp sulfate = 5.505mg of d-amphetamine base (73.4% by weight)
2.5mg of l-amp sulfate = 2.7525mg of l-amphetamine base (73.4% by weight)
5.0mg of d-amp saccharate = 2.8mg of d-amphetamine base (56% by weight)
2.5mg of d-amp aspartate h2o = 0.5925mg d-amp base (47% by weight)
2.5mg of l-amp aspartate h2o = 0.5925mg l-amp base (47% by weight)
total amp base = 12.24mg (8.9mg d-isomer, 3.34mg l-isomer)

This is based on the knowledge that aspartate monohydrate forms salts with only 1 ion, and the prediction that saccharate forms salts with 2 ions (and the math shows this is correct as the predicted amphetamine base matches the monograph value).
