Like the title says, how do you know? I'd been doing well, but lately I feel like I'm falling back into some of my non-productive, ADD habits, and am wondering if upping my dose would help. I'm due to see my doc in a few weeks, so I'll also be discussing it with him.

I knew when to increase mine when I couldn't even tell if I had taken it or not. When I reverted to being severely hyperactive, doing multiple things at a time, and finishing none of them because I would forget entirely that I was even doing them. When I was frantically tearing apart my apartment looking for my cell phone while talking on it. When I was bringing a client of mine change for a $20, but then spent it on the way. If you're lapsing back into your old ADHD mode of existence, I would definitely talk to your dr. about increasing the dose. I've only been taking adderall since April, but I think it's fairly common to have to increase it several times in the beginning until you find the optimal amount. I don't have enough experience with it to offer any advice for the long term, and individuals vary greatly. However, if you discuss these issues with your dr., hopefully s/he will work with you so you can go back to being more productive.
