I saw someone else asking this question but I was moved to a different doctor and starting in March I was on Adderall and it didnt work...then my doctor switched me to Vyvanse which did nothing either...so I went to a different doctor and Concerta is what I am on now....54mg....it seems to work when I take it at 9am then dies off at 3 then I start crashing...I need to work till at least 7 most nights or if my medicine wears off I dont want to talk to anyone at all...I just want to lay around....does anyone recommend a second dosage in the late afternoon..I want to talk to my dr about it but they might see a "drug seeking" behavior...which is not the case.

Tell you doctor exactly what you've said here, give the details of the crash, the timing, how it feels and all ... he'll most likely give you a booster. No need to fear the druggie thing. Plenty of people add a short-term dose to the extended release pill.

Being alone when the medicine wears is much easier. I start deep breathing. But when I am around people, it is a little confusing.
My pdoc told me I would probably need what he called a "bump" dose. I haven't started with this yet, but see where it would be helpful. Good luck!
It's very common for these long lasting meds to not last all-day like the marketing material promotes. Your doctor may have a couple of options such as:
1. Upping your dosage to 72mg
2. Adding a smaller, second dose of Concerta to take a few hours after taking your 54mg, maybe around lunchtime.
3. giving you a short-release ritalin prescription, although if you are only getting 6 hours and you need a good 4-5 hours more coverage that probably won't do it.
