very useful for when you need to quantify to your pdoc your functioning [or lack of it] level or stim med effectiveness, dosage, and side effects

i use the weiss functional impairment chart off and on. good to keep tabs on the bigger picture cause i'm a day-to day person with no clue about my week to week or month to month unless i use these.

There is also the DIVA, which is a new and free diagnostic interview used a lot in Europe for diagnosis of adults. It has questions both about childhood and adult symptoms, and is available in many languages.

I'm not really sure how much it's used as a self report tool, but it probably fits here anyway.
