I'm on 54mg. I worked my way up to that. Took 18mg, 27mg, and 36mg before settling into 54mg. This was only over a month long period though, under a doctors guidance of course.

On 18mg I knew I needed more because it only lasted for 4 hrs. On 27 it lasted 6 hrs, and focus was better, but I still didn't feel like I was functioning as well as I would have liked to. On 36mg when I first tried it I was hyper-focused, so I was not going to continue assuming the dose was to high for me. My doctor suggested to wait a week, since I wasn't miserable, and was seeing other positive effects. At the end of the week the hyper-focus was gone. I got about 7 hrs out of the 36mg.
I still was not where I wanted to be though and was having some rebound issues. My doctor suggested 54mg to see if it helped. It did. I'm functioning as I would like to now. I get about 8-9 hrs out of a dose, and the rebound issues are gone. I know I don't need more because I'm getting all the benefits that I initially wanted, plus no side effects. I know I don't need less, because I've had less, and wasn't happy with my functioning level.

Here is something funny! I was searching for an answer to this very question because i was just prescribed the med AGAIN cause I couldn't sleep the first time.

I just noticed after reading the entire thread, that it was MY post I was reading!

WoW! 2006 and I just found my own thread in 2008 while googling about concerta! I feel stupid!

Well, I've tried 72, 54 and now the 36s

On the 36s I still get that (brain turned off while trying to keep focused.. kinda of go blurry and lose visual focus.. hard to explain) feeling.

Didn't have it on higher dosages.

Also, notice that I am starting to skim sentences instead of reading the entire thing.

I'm thinking 54 might be the right dosage.

I have a cold, home that didn't mess up the variables too much.

I'm on 36mg of Concerta. Worked OK for about 2 weeks, now I'm back to being very ADD. Can't concertrate at all. I see the doc on Monday for an increase because I didn't want to double up on 36...seems like a big jump

Anyhow, I found this:

You know how I knew my dose was low enough? Because when meds work, at night I am brain dead and able to sleep...when they don't work I get awful bouts of insomnia!! Last night I had <4 hours sleep...same with the night before....makes my ADD even worse!!! That's because I feel the need for coffee in the afternoon because I am so out of it...

Go figure...an ADDer must take stimulants to sleep well at night...how counter intuitive!

Anyow, that link above says the mean dose for hte study inthe end was 67mg

Since I can only speak as to the behaviours I saw, I can tell you that in her case the dose was not working as effectively or for as long as it had been previously and her focus was back to where it was before she was on medication. She was back to the nonstop talking and nonstop movement as well.
The reason for the changes according to what I have been told wasn't tolerance related. For her she went through a big growth spurt where she put on a inch of height and went from 37.5 to 41 pounds in a matter of 6 weeks time. The specialist she sees would not put her on anything higher than the 18's due to her weight he had concerns that anything higher would be too much for her body to handle. Up until recently the 18's were working great for her so there was never any need to increase the dosage so I wasn't worried about it.
I called for an earlier appointment and found out the doctor is on a sabbatical until early September but was able to talk with his nurse. She felt the weight gain was the reason and advised me to take her to our regular doctor for now and have him do a trial on the 27's and then the usual if it works have him give you enough until your next appt and make sure his office lets them know if she stays on that dose so he is aware of it when he sees her in the fall for her next med check.

Perhaps it's an indication that I am a slow metaboliser of drugs but I would often still feel the effects of instant release ritalin at night time and early in the morning before my first daily dose. Concerta, however, hasn't been quite the same. Whilst there haven't been any side effects at all, I feel understimulated for extended periods of the day and its coverage doesn't seem to last 10-12 hours.

I think I would benefit a great deal more if I were taking 54-72mg of concerta in the morning and an IR form of ritalin in the afternoon. Unfortunately I believe my doctor is reluctant to add anything to a prescription of concerta.
