Question New on Adderall-question on effect

Hello, I am new on adderall about 3weeks... I was having trouble staying awake at work due to a sleep issue. I tried an adderall 20mg which was given to me by someone and it worked WONDERS as far as keeping me up and improved my work preformance. When this happened i decided to discuss it with my DR and he put me on Adderall XR 15mg. He had me try this for 2 weeks and during this 2week period I noticed no difference in my engery and alertness. and also my Co-pay for the XR was ALOT and my pharmaciest told me that if i switched to the regular adderall my copay would only be $10. VS $50. I went back to my DR for my 2 week med check and told him what was happening so he changed my prescription to Adderall 10mg to be taken twice a day and since i work nights once at around 11:30pm than another about 4-6hrs later. I got the prescription filled which was NOT EASY as the state that I live in Northern VA has an extreme shortage until at least Jan, well CVS. Walgreens, Riteaid, ETC are short. I did find it at a specialty pharmacy.....anyway back to my issue. I started on the adderall 10mg took 1 and NOTHING no difference in my alertness or my energy...took another one 4hrs later still nothing...on my second night of working I was so tired and falling asleep on the PHONES that i decided to try 2-10mg which = the 20mg and STILL NOTHING!!!! I even went as far as taking 4 at a time the 3rd night and STILL nothing...Im back to only taking it the way the dr told me but im at the point were im like why even bother taking it....does anyone have any idea why when i took it the 1 time it worked so GREAT but now it wont do anything and have only been taking it for actually less than 3weeks...please if anyone can tell me what I need to do...do I need a higher dose in 1pill form for it to work....any ideas??? or can anyone tell me if theres another medication similar to adderall that i might have better effects with that i can discuss with my dr. Im gonna call him for an appt for mon and explain whats going on.

Adderall can be really good for narcolepsy (falling asleep when you definitely got enough sleep), but will only make other sleeping problems worse. It can prop you up when you're tired, like mega-coffee, but it doesn't "work wonders" without a really good night's sleep.

20mg of Adderall XR at 8 am will have me up until 3-4am easy. And if I get less than 6 hours of sleep it basically does nothing helpful.

What kind of sleep issue is it?

I know the answer! Let me guess... your new script is filled with sandoz/eon labs adderall. I have had the same issue with this medicine after using BARR for a year. The barr gave me energy like an energy drink w/o the jitters but still allowed my mind to be clear and gave me awesome motivation. This new brand gives me clarity and mental focus but NO energy and only a smidge of the possibility to be motivated. I have been supplementing with about 4oz of coffee. I actually like the clarity of this adderall and the fact that it does not make me hyperfocus but I need the push and energy of the other! I wish I could have just a few of the BARR to supplement these butttt with the shortage who knows when that will happen. I have considered switching meds but don't know what to try. I hate ritalin and vyvanse does not get close to my old friend BARR. le sigh

If you have trouble staying awake, Nuvigil is also used to treat narcolepsy and shift work sleep disorder. Unfortunately, it is quite expensive...but maybe it depends on your insurance. I had a few samples because my pdoc at the time did not want to re-prescribe me Adderall due to a manic episode earlier this year...In my experience, it does help with alertness (for 10-12 hours too, if I remember correctly), but not necessarily concentration (although the peak in alertness can still be helpful indirectly, or in other ways in terms of improving quality of life)...A couple of times I was physically fatigued and wanted to take a nap, but was not able to. I agree that Adderall is not quite as effective if you haven't had a good night's sleep...

Modafinil (Provigil) is related to Nuvigil and older, so it might be less expensive.

However, I'd consider addressing the sleep issues first. See a sleep specialist if necessary.

That I can totally relate to. I know Adderall can be used to treat narcolepsy and shift related sleep disorders, but you're the first person I've seen here talking about it.

Sleep and Adderall are very closely intertwined, and how much sleep you do or don't get has a major impact on the way that Adderall affects you. This I know from personal experience.

Personally, I think your doctor has the treatment priorities backwards - it's more critical to get you on an effective sleeping cycle before you deal with the awake part. First of all, if you're getting adequate sleep, being awake when you need to be is easier. I've worked graveyards, swing shifts, night shifts, early morning shifts - all over the clock. Because of my personal 'normal' sleep schedule, nights work better for me, but several days of no sleep is disastrous, regardless. Like you, I'd go several days and nights with no sleep, catch a few hours, and do it all over again. After a while, it takes a major toll on your health. Even if it's the way your body 'works' it isn't healthy.

Adding Adderall to sleep deprivation has some strange and subtle negative side effects for me, like completely eradicating my appetite. I never feel thirst OR hunger. It also becomes difficult for my body to regulate its temperature - I'm either freezing or roasting. And whether you feel the effects or not, it WILL affect your ability to sleep, especially if you are using it to treat a sleep disorder rather than ADHD. For me, it combats the hyperactivity, and has a sedating effect at night, rather than a stimulating one, but without ADHD, you're only getting the stimulation.

The reasons and ways that you're increasing your dose can be dangerous, and have some serious, negative health impacts. I second the suggestion that you see a sleep therapist, or at least talk to your doctor about the importance of healthy sleep rather than staying awake. Adderall carries a significant risk of addiction. When you take Adderall for ADHD, you're correcting an already occurring chemical imbalance. Without ADHD, you're adding a chemical to push your body beyond its normal state, into an unnatural one. That is going to increase your addiction risk and the likelihood of negative side effects.
