"chosen" behavior problems vs. decreased effectiveness of Strattera??

My son has been on Strattera 25mg for barely 1 calendar year - he's 7 now. Over the past.... 6wks or so, he's declined considerably in school & we've had problems at home also. The teacher is now tracking his daily progress with a sheet broken down by set daily activities. This has only been done for 3 days so far. I had asked if the "pattern" was markedly worse in the afternoon, or whether the time of day was irrelevant.

It seems that the things he's doing is trying to do anything that's "fun" or more fun than his schoolwork, which the school work is the worksheets & things he has to focus on & sit still in order to complete. He jumps out of his seat, blurts out things, takes toys & tries to hide them so he can play with them rather than doing his schoolwork. At home, he more hyper & impulsive, constantly fidgeting, making noises, talking non-stop, can't stay in his seat to eat dinner, etc.

The dosage of Strattera has not changed in this calendar year. I know the meds are not supposed to be a miracle cure, but to give him that added focus or lack of impulsivity in hopes he makes better choices.... I've made an appt with his therapist, but it's not for 2wks, for a re-eval since it's been about 16 mo. since he saw her. From what I've read on some of these threads... Strattera's effectiveness goes down considerably, or the brain compensates in order to offset the balance of meds vs. hyper/impulsive, as time goes on - which I've been unaware of up to this point.

Does all this sound like it's a medication problem, that Strattera's effectiveness has decreased considerably, or he's making poor choices throughout the day?!?!?!?!?! Maybe I've scheduled an appt with the wrong doc & his PCP needs to adjust his meds...

Is it me, or do most great ideas seem to come while in the shower?!

Me-No problems-will be 30 in April.
Son-Now 7, Dx age 5 1/2, ADHD-hyperactive/impulsive, no LD or BD or OCD or ODD that we're aware of
Daughter-no problems-3 1/2
Amazingly Wonderful hubby of 8 years.
Hubby & his 2 brothers-history of undiagnosed ADHD-impulsive-no meds ever

Has your son gained any weight recently. It seems that the Strattera may just need to be adjusted somewhat.
