Re: List of ADHD Medications - On-label and Off-label

Dextromethamphetamine seems to be the best medication available for ADHD-PI and "SCT." A second best might be Dextroamphetamine

Dextromethylphenidate seems to be the best medication available for ADHD-C and ADHD-Predominatly Hyperactive. Methylphenidate as second best maybe. Cocaine and Nicotine would also be useful.

Stimulants and Wellbutrin seem to increase anxiety
Tricyclics, Strattera, Guanfacine/Clonidine not so much

Re: List of ADHD Medications - On-label and Off-label

Beta blockers to reduce bodyload on PNS created by stimulants
L-type calcium channel blockers to reduce stimulant-induced anxiety

SSRIs to handle comorbid depression
SSRIs or KLONOPIN to handle comorbid anxiety. Other benzos possible, but slow cognition a lot.

hmm...BuSpar and beta blockers might reduce stimulant-induced anxiety

Obscure stimulants:

Dimethylamphetamine has occasionally been found in illicit methamphetamine laboratories, but is usually an impurity rather than the desired product. It may be produced by accident when methamphetamine is synthesised by methylation of amphetamine if the reaction temperature is too high or an excess of methylating agent is used.
Phenmetrazine
Ethylamphetamine
4-Methyl-aminorex
Methcathinone
Cathinone
Fenethylline
Aminorex
Benzylpiperazine

More:
Diethylcathinone
Dimethylcathinone
Phentermine
Sibutramine
Crack Cocaine

Don't worry, effectiveness of obscure stimulants for ADHD will be discussed eventually.

Contrapunctus is an unknown quantity at this point
Re: List of ADHD Medications - On-label and Off-label

Pyrovalerone and Prolintane look like good possible ADHD meds (relatively long lasting DARI/NERI's). Pyrovalerone is still available in the US, and it is a C-V medication! Unfortunately, I have never heard of it being prescribed for any purpose.

Also, diethylpropion (Tenuate) could possibly be used for ADHD. Aminorex would also be a good treatment for ADHD, having good ratios of NE/DA. 4-methyl-aminorex is less potent by weight when compared to Aminorex, but may have unique pharmacological properties. From my experience with Pemoline (cylert), I found that the oxazoline class had a very low incidence of side-effects, as well very little efficacy as an anorectic (which is good!). It was certainly the most "nootropic" of the stimulants.

Re: List of ADHD Medications - On-label and Off-label

Good find Contrapunctus. Unfortunately, Pemoline isn't used much. It is still available as a generic, although Brand-name Pemoline "Cylert" has been discontinued. I haven't found pemoline's effect on the liver to be so concerning. Many others have similar effects. My feeling is it should be used a lot, with careful monitoring of the liver.

I got caffiene in post 4

Good reminder about sudafed. I just hope sudafed PE (which doesn't contain pseudoephedrine) doesn't replace the original one. Otherwise, the original will be discontinued.
